Lockhart Home Health begins with an in-depth initial appointment with Aliah Lokhart Bsha, MS, The owner, or a chief Care Manager. During this comprehensive meeting, we will:
- Review all available care and support options, including personal care, medication management, mobility assistance, companionship, and specialized services for specific health conditions.
- Discuss in detail the levels of care packages, outlining what each tier includes and how they can be adjusted.
- Collaborate with you and the client to design a fully personalized care plan that addresses unique needs, daily routines, medical requirements, and personal preferences.
- Answer any questions regarding scheduling, caregiver qualifications, and ongoing communication.
Please bring documentation of the diagnosis, current medication regimen, and any relevant medical history to ensure we have a complete understanding of the client's needs.
Our process includes two meetings:
1. The first meeting is held at our office or another agreed-upon location, allowing us to establish rapport and clarify your goals in a comfortable setting.
2. The second meeting takes place at the client's home or place of care, where we assess the environment for safety, accessibility, and suitability of services, and make recommendations for modifications or additional support as needed.
